Insurance Declaration Form

All LIT students going on field placement must complete the Insurance Declaration Form prior to the start of the placement. Please follow these steps:

1. Download the Insurance Declaration Form from the "Attachments" section below.
2. Complete and sign the Student Declaration section.
3. Upload the form, along any with other required documents, to the Supporting Documents Submission Form .
