Field Placement Important Dates

Summer 2026

• Attend Field Placement Orientation & Information Session. Please refer to the schedule posted on your BlackBoard Course for MFP501 
• Submit Field Application Application: May 10, 2026 @ 11:59 PM ET
• Complete a minimum of 10 full business days of field placement within the semester
• Submit attendance, student evaluation, and host evaluation within two business days after completion of field placement. Keep copies for yourself.

How to Submit Documents

 1. FIELD PLACEMENT APPLICATION

Instructions and requirements for applying to field placement:

1. Have completed all professional requirements of the academic program other than field placement by the end of the semester; please submit an application even if you don’t meet the requirements
2. Complete the field placement application form - link to be provided to you in BLACKBOARD.

Supporting documents are required along with this application

(MUST BE SUBMITTED ON BLACKBOARD):

1. Submit an error-free résumé -Field placement involves demonstrating an ability to prepare a résumé in accordance with professional legal standards.

2. Unofficial transcript -  can be viewed and obtained by selecting the “Academic Records” tile in Student Home. There is no fee for unofficial transcripts. 3. WSIB Insurance Form - The College requires all students to complete an insurance declaration form. This document is mandatory in order to be eligible and considered for a placement. Complete only the Student Declaration section. The form can be found at the "Attachments" section at the bottom of this page.

Electronic Review Verification: Medical Clearance+ Vulnerable Sector Check + Professional Conduct Agreement

In order to fulfill the terms and conditions of your field placement offer, the medical clearance, Professional Conduct and Vulnerable Sector Check documents must be uploaded to SYNERGY GATEWAY.

All students in MFP501 will receive an email from Synergy Gateway. Please check your inbox to see if you have received your Synergy activation link. The email could be from (helpdesk@synergygateway.com). Please do not report it as a Phishing email.

Once you receive the email, please follow the steps provided and ensure you access the link within the 72‑hour time limit, as the activation link will expire after that period.

If your link has expired, please use the “Forgot Password / Resend Password” option on the Synergy login page to receive a new activation link.

After successfully logging in, go to the Important Documents folder, download and sign the MFP Professional Conduct, and upload it to Synergy. This document is required for you to receive a pass clearance.

Important notes:

There is an associated fee to submit your documents on Synergy Gateway. This fee helps ensure that your documents, Medical Clearance, especially your Vulnerable Sector Check (VSC), which may contain confidential information are protected and securely stored.

These sensitive details should not be accessible to Seneca faculty or staff, and Synergy provides a secure, third‑party platform for proper handling and verification. Appointments should only be booked after you have received and uploaded your Vulnerable Sector Check (VSC).

Documents to be submitted on Synergy:

Vulnerable Sector Check are required for individuals who will be working with vulnerable populations, such as children, youth and adults with disabilities.

MFP Professional Conduct Agreement - This agreement is designed to help provide you with some awareness and success tips that can help you achieve your academic goals and work towards successful completion of your program. You are encouraged to review and sign this agreement with your Field Placement Coordinator/Liaison prior to commencing your field placement. 

Medical Clearance -  You can download the form from the "Attachment" section of this page. If you require updated screening and/or vaccines, this can take time. Once completed, you need to upload this to Synergy. 

BOOKING AN APPOINTMENT ON SYNERGY: 
Once you have completed the Medical Clearance, Obtained your VSC, and signed the Professional Conduct Agreement, book an appointment on Synergy Gateway using your secured username and passcode.

Booking early before you have the required documents may result in missed appointments and could cause unnecessary issues with your account.

2. COMPLETION OF FIELD PLACEMENT

Submit signed copies of these documents within two business days after completion of field placement:

• Student Attendance (excel) – must be filled electronically, print, have your host supervisor sign. Keep a copy for yourself.
• Student Evaluation (excel) – must be filled electronically. This is your analysis of your experience.
• Host Evaluation (excel) – please provide this form to your host to complete. The host supervisor may choose to hand it to you, or they may submit it directly by e-mail to the Field Placement Coordinator; this form is our proof that you met the requirements and fulfilled your obligations of field placement.
