What if I choose not to get the Flu Shot or any other vaccines? | School of ECE Field Placement

Should you choose not to take the influenza vaccine or other immunizations, you must complete the Statement of Conscience or Religious Belief Immunization Form and present it to Concise Health and your placement. This affidavit must be signed and stamped by a Notary or Commissioner for Taking Affidavits (e.g., a lawyer). 

 Some options for Notary and Commissioner services: 

• Toronto Notary Public 

• Notary Pro 

• Notary Assist Inc (online services) 

These are suggested options. Students may choose other organizations. 

NOTE: Seneca has no affiliation with these organizations. 

Please note that students that do not receive the flu shot will need to leave placement if there is a flu outbreak at placement. Students will be permitted back once approved by Public Health. Student must make up missed hours/days.
