Student Profile Questionnaire helps our field placement team match you with a field placement. This can be completed before your complete Step A.

Prior to completing the Student Profile Questionnaire have the full names of your previous field placement organizations. Refer to your previous Time Sheet and Task Tracker and Reflection of Learning Field Placement for this information. 

Please complete the Student Profile Questionnaire for Fall 2026. The Field Placement Team will see your information as soon as you submit your response.

These questions help the Field Placement Team find the best suitable option.

Answer all questions accurately with the correct information. Please include the accurate full name of each organization you have had field placement experiences.

Missing or inaccurate information provided may lead to delays in starting field placement
